Collection of Learning Materials and Important Documents (released on 8-9-2020)

Please note the following arrangement:

 

(A) Overall Arrangement

Besides preparing a folder to file all learning materials, if your child did not come for the collection in late August, please be reminded that they should bring the fee listed below to school.

(B) Learning Materials

Students of different classes will receive different set of learning materials; class teachers will inform students of the details this week.

 

(C) Declaration Form

To keep the school informed of students’ health condition, please complete the “Declaration form for travel history and health status of students” and ask your child to return the form to his/her class teacher on the first day when he/she attends face-to-face lessons resume. In case your child is unable to come to school collecting the hardcopy, please download, print and fill out the form.

 

(D) Temperature Record Sheet

Starting from the resumption of face-to-face lessons at school, parents are required to check your child’s temperature before he/she goes to school every day. Please fill in and sign on the “Temperature Record Sheet”. Your child should pass the completed sheet to the class teacher every morning. In case your child is unable to come to school collecting the hardcopy, please download, print and fill out the form.

To avoid overcrowding, parents do not need to accompany students for the collection; or parents may collect the items for your child.
